Jacob L. Carlson, age 29, of Spencer, Iowa, and formerly of Sioux Rapids, died on Saturday, July 11, 2015, at the Hawkeye Care Center in Sioux Rapids.

Memorial services will be held Friday, July 17, 2015 at 3:00 p.m. at the First United Church in Sioux Rapids. Private family burial will be in the Lone Tree Cemetery in Sioux Rapids. Visitation will take place Friday, July 17, 2015 at the church beginning at 2:00 p.m. until the time of the service. The Sliefert Funeral Home in Sioux Rapids is in charge of the arrangements.

Jacob Lynn Carlson, the son of James and Lorene (Lori) Wise Carlson was born in Storm Lake, Iowa, on April 16, 1986. Jake lived his youth and received his education in the Sioux Rapids community, graduating from Sioux Central High School. He continued his education at the Iowa Lakes Community College in Emmetsburg, Iowa where he received two Associate Degrees.

Jake courageously battled many health challenges throughout his lifetime. At the age of 20 months, he was diagnosed with leukemia. He received treatment and the cancer had gone into remission three times. In 1994, he received a bone marrow transplant. Jake was a double lung transplant recipient in 2006. His health gradually declined the past several years. When his health permitted, Jake had worked at Wal-Mart and the Sioux Care Center.

Jake was a member of the Covenant Fellowship in Spencer, Iowa. He was one of the first participants in the SYC (Special Youth Challenge). Jake truly enjoyed deer and turkey hunting along with fishing. His niece, Layla, held a special place in his heart.

Jake is lovingly remembered by his family which include his father, James Carlson of Sioux Rapids, Iowa; brother and sister-in-law: Joshua and Sarah Carlson of Cedar Falls, Iowa; grandmother: Darlene Wise of Linn Grove, Iowa; many aunts, uncles, cousins, extended family and friends.

He was preceded in death by his mother, Lori; aunt, Denise Wise; and his grandparents.

In lieu of flowers, the family requests memorials in Jakes memory to SYC and the Heart Connection Camp in Boone, Iowa.

May Jake’s life and memory continue to be lived out through his family and friends.
